Access to Health Records & Consumer Rights
In compliance with Texas House Bill 4224 and Texas Health & Safety Code, Chapter 181, South Texas Counseling Agency is committed to protecting client rights and ensuring transparency regarding access to health records.
How to Request Your Health Records
Clients may request copies of or access to their health records by submitting a written request to South Texas Counseling Agency.
Requests may be made by:
- Sending an email to: sotxca@live.com
- Requesting and completing a written health records request form
All requests will be handled in accordance with applicable state and federal laws.

Filing a Consumer Complaint
Clients who believe their rights related to health record access have been violated may file a complaint with the Office of the Texas Attorney General.
